What is Fallen Heroes? How do we define this theme?

The above photo might be the very first thing which crosses your mind when you hear the phrase, 'Fallen Heroes'. Most would assume it's the falling of Superman against Kryptonite, or Prince Charming losing to the Wicked Witch.
However, nothing in this world is black and white.
What makes a Hero?
Generally, the individual words in the phrase are commonly used and easy to understand. If we think of the word Hero, what comes to mind first is of course; the conventional cartoon hero.
Pew Pew Pew!
However, by definition we know that a Hero is someone who is looked up to, usually for when they have courage and:
Successfully faced/conquered a difficult challenge
Achieved a previously 'impossible' feat
Stands up for the rights of the weak
Therefore, a hero may be a war hero such as soldiers, doctors, firefighters, janitors and the average parent! Sometimes, this term is used for animals as well, as we might glimpse from these headlines:

So, what is a Fallen Hero?
How does a Hero fall?
We ourselves are heroes no matter who we are. We are heroes for our family, friends, community, nation and the most important part is we are heroes to ourselves. Whether we are students, teachers, or even cleaners!
